Today is my grandmother's 88th birthday. I was on the phone with her for 50 minutes, and around the twenty minute mark she transitioned into thinking that she was talking to my younger brother, of whom she is apparently a huge fan. There was a very awkward twenty minutes about how I'm a great father to my nephew and a great husband to my lovely sister-in-law. She is less of a fan of my brother-in-law, and made me promise to be a strong role model for my other nephews.

I'm getting to the end of a doozy of a 12 hour shift in which my patient tried really hard to go into a full respiratory collapse. He's got a tracheostomy through which I've had to suction - constantly - overnight and he's kept plugging off because of all the gunge in his chest. Watching a patient struggle for breath through their throat while you're trying to clear it of shit is not amusing.

Don't mind saying it - and I apologize for the language - but I'm fucking toasted.

Don't smoke.

My reflections are simple if you are basing all your medically and socially based decisions on her tweets as she notes you get what you pay for.

Most people who have a brain and understand what twitter has of value is to inform, humor and vent. So it is not an advice column at least hers isn't.

The many comments that follow are just so self righteous.
Because everyone is so stupid and will trust a follow up tweet too.
I love people who treat others with superiority and contempt for their own good. Tiresome.
